Breakthrough Surgical Adhesive Dramatically Reduces Fluid Accumulation After ... - Forbes

Knowing that a loved one has come through a plastic surgery procedure is a relief. Seeing them post-surgery sprouting drainage tubes from various parts of their body is another story entirely.

Drainage tubes have long been one of medicine?s necessary evils. After plastic surgeons complete their procedure and are about to close the incision, they?re looking at an open wound. Ideally these layers of tissue, which the surgery has disturbed, need to be securely ? really securely ? reattached to each other and to the inner portion of the dermis before the wound is stapled or sewn. Without this level of adhesion, fluid will accumulate in the wound space.

?Fluid build-up is enemy number one because, when the tissues gets separated by fluids, you don?t know how it will react,? says Dirk F. Richter, a plastic surgeon and chairman of the plastic surgery department at Dreifaltigkeits Hospital, in Wesseling, Germany. When strong tissue adhesion is missing ? and fluids start accumulating ? drainage tubes are the only recourse. The challenge is even greater with large-flap surgeries: think stomachs undergoing tummy tucks.

However, a new technology which became commercially available in Europe two months ago, may change all that.

Over the last two months, Richter has been using TissuGlu Surgical Adhesive, a product that may be the biggest breakthrough in plastic surgery in years. ?It?s what we?ve been waiting for,? he says. ?Surgery, which is always a trauma, produces swelling. And the swelling produces fluid in the tissue vacuum. Fluids are the best environment for germs to produce infection.?

TissuGlu holds the layers of tissue in place and prevents fluid accumulation, thereby reducing the need for drainage.

An adhesive developed for plastic surgery procedures, TissuGlu consists of biocompatible material ? a lysine-derived urethane polymer adhesive that performs ideally in a moist environment ? that can be broken down and assimilated back into the body. TissuGlu?s magic lies in the strength of its adhesive bond, which research has demonstrated to be five times stronger than any commercially available products used in soft-tissue surgical procedures.

?The faster an adhesive can seal the wound ? so that tissues can?t be pulled apart ? is what controls the accumulation of fluid,? says Richter. In surgeries where he used TissuGlu ? the product is delivered in a grid pattern, three drops at a time onto the open wound, through a handheld applicator that automatically releases the product ? ?fluid accumulation was dramatically reduced.?

Up to this point, there has been no synthetic non-toxic adhesive on the market able to seal layers of internal tissue strongly enough to each other to keep fluid from accumulating, says Patrick Daly, 47, founder and CEO of medical product developer Cohera Medical Inc., which created TissuGlu.

For the past 100 years, Daly explained, the medical establishment has relied on rubber drainage tubes to remove accumulating post-operative fluids. ?The tubes stayed in the patient for as long as fluid came out of them, which could be as long as 10 days,? he says. ?Tending the tubes are inconvenient and painful to the patient.? Even worse, he adds, between fifteen and 52 percent of patients have medical complications ? the drainage tubes may become infected, clogged or leak.

Even after physicians remove the drainage tubes and the patient is sent home, seroma ? fluid build-up in dead space after the drain has been removed ? may still occur, which results in the wound area ?becoming as puffy as a water bed,? says Daly. Then, it?s back to the doctor?s office for fluid removal and perhaps a reinsertion of the drainage tubes.

The College of Physicians and Surgeons of Nova Scotia has launched an education campaign for anyone wanting cosmetic surgery or surgical implants.

The advertisements, running in local newspapers, encourages people to ask the right questions before getting a cosmetic procedure done. The college also provides a list of questions patients can take to a doctor or a non-medical cosmetics salon.

"On the one hand it's medicine, on another hand it's something a little different," said Dr. Gus Grant, the registrar and CEO of the College of Physicians and Surgeons of Nova Scotia.

"We don't want that sense of it being different from your typical doctor's visit to prevent patients from getting what they're entitled to, which is an informed opinion about the nature of the decision."

The education campaign was prompted by a high-profile case in Ontario, where a 32-year-old woman died after a liposuction procedure went wrong.

Dr. Richard Bendor-Samuel, a plastic surgeon in Halifax, said a public awareness campaign is needed with more general practitioners making home visits for Botox parties and dental surgeons doing face lifts.

"I've seen burns from facial procedures that were done that you wouldn't really expect a burn for pigmentation," he told CBC News on Friday.

The College of Physicians and Surgeons of Nova Scotia has no jurisdiction over spas and salons where the staff are not doctors. It's an issue the college says it's starting to think about.

"A number of these new cosmetic procedures are invasive procedures that carry risks with them that are inherently, by feel, medical in nature," said Grant.

Bendor-Samuel said in the end, the provincial government must decide what work should be done by doctors.

"Physicians can't regulate the dentists or the aestheticians or just the general people who are opening up private practices, private clinics that have nothing to do with medicine but are doing semi-medical procedures," he said.

The stunning 53-year-old actress admits she would consider having a little nip-and-tuck if she felt she needed it but has blasted people who go too far claiming they make her "uncomfortable".

She told America's Elle magazine: "I'm all for a little something here and there - fine. It doesn't matter to me if people have plastic surgery or they don't, or if they do Botox. But when people don't look like themselves anymore, that's when you kind of go, 'Oooh,' and it's kind of sad. Before allowing any doctors to perform a plastic surgery procedure on you, you should check out their qualifications, the American Society of Plastic Surgeons (ASPS) announced today in a new public safety campaign. This follows a series of reports on botched up plastic surgeries, carried out by non-board-certified plastic surgeons.

Malcolm Z. Roth, MD, ASPS President, said:

"Patients are getting injured, some are dying during procedures performed by non-board-certified plastic surgeons. We want patients to understand what to ask their doctor and what to look for so that they can maximize their chance of a safe and successful procedure."

White Coat Deception - a term used by plastic surgery industry leaders. If a doctor is wearing a white coat, it does not necessarily mean that he/she is a qualified plastic surgeon.

A white coat is just that - a coat. It is not a board certification in plastic surgery

Dr. Roth said:

"There is a misconception among consumers that as long as a doctor is certified in a medical field that he or she is qualified to practice plastic surgery. This is absolutely wrong and it is dangerous for patients. If you are considering a plastic surgery procedure it is critical that your doctor is a member of the American Society of Plastic Surgeons. This is the best way to be certain that he or she has completed the necessary training and meets certain standards to practice plastic surgery."

By law, in the USA any doctor who has a medical license to practice in any field of medicine can legally perform plastic surgery. They do not have to be board certified in plastic surgery. Texas, Louisiana, Florida and California have Truth-In-Advertising requirements which demand that medical providers be clear about their training.

Dr. Steven Teitelbaum, MD, an ASPS Member Surgeon who practices in the Los Angeles area, said:

"Today when you get your medical license you can practice in any medical field that you choose. What should happen is that every state medical board should say, 'if you're trained in pediatrics you are allowed to practice pediatrics and if you're trained in orthopedics you can practice orthopedics.' But, unfortunately, most state laws and regulations enable some physicians to drift into the practice of plastic surgery without proper training and certification. It has become increasingly apparent that some physicians fail to use proper judgment and enter into practice areas beyond their core training."

Dr. Teitalbaum explains that a growing number of patients are coming to him to have botched plastic surgery procedures fixed, all of them carried out by non-board-certified doctors. He quoted the case of one of his patients, Dinora Rodriquez who had a breast implant surgery from a non-board-certified plastic surgeon. When she woke up after her operation, she found that the implants were merged together. Her surgeon had also decided to do a procedure on her eyes without asking her first. She is unable to close her eyes properly now.

Dinora Rodriguez said:

"It was a terrible experience waking up from surgery and seeing that this had happened. I didn't know to check my doctor's qualifications and I regret it."

The ASPS says that when you are checking out your plastic surgeon:

  • Ask whether he/she is board certified in plastic surgery by the American Board of Plastic Surgery
  • Seek out a certificate in the doctor's office that is endorsed by the American Society of Plastic Surgeons
  • Click on "Find a Surgeon" at www.plasticsurgery.org. See if your doctor is listed there.

Plastic Surgery for Men: 'Just Take a Little Off the Top'

BALTIMORE, Oct. 6, 2011 /PRNewswire/ -- Women get the lion's share (87 percent) of all the surgical and non-invasive cosmetic procedures in the United States. That's why most plastic surgery advertising is still directed toward them.

Plastic surgery for men, however, is on the rise. According to the American Society of Plastic Surgeons, male cosmetic procedures were up 2 percent last year. Statistically, men preferred surgical solutions rather than repeated non-surgical treatments, which bucks the national trend. The latest cosmetic surgery research has uncovered an increase in the number of people over 65 going under the knife. A study done in the UK shows that plastic surgery clinics have seen an 18 percent increase in requests for facelifts according to reports. Some feel this might be due to the influence of celebrities like Joan Rivers who openly discuss their cosmetic procedures.

He is an associate clinical professor at USC, where he has recently taken on the role of co-director of the Aesthetic Surgery Division.

Stevens has persisted in being a leading voice for aesthetic medicine, not only as a mentor to other surgeons but their teacher. He stands in front of a long list of accomplishments, most notably the cohesive breast implant (aka, gummy bear breast implants), the Mentor CPG and the Sientra highly cohesive breast implants, the Mommy Makeover, the Laser Bra, and simultaneous mastopexy augmentation.

WATCHING EVOLUTION

Stevens holds as a priority the open discussion of what is in store for the future plastic surgeon. He actively promotes the viability and continuation of cosmetic plastic surgery by plastic surgeons.

?Turns out that 72% of board-certified plastic surgeons make the majority of their income from doing cosmetic plastic surgery,? Stevens notes. ?Now, there?s a disconnect here. If 72% of the surgeons are making more than half their income from cosmetic surgery, then why aren?t we teaching cosmetic surgery??

Stevens believes there is a legacy being created by his specialty, and he takes it personally. ?I want plastic surgeons to remain significant in this space,? he says, ?and the only way that we?re going to do it is by teaching the residents, even if they don?t do it for a lifetime. I?m not doing craniofacial surgery, but I had to do craniofacial training. I?m not doing neuro surgery but I did neuro training. And I respect the surgeons in the other divisions.

What about the practitioners against doing cosmetic surgery? ?They have this sort of distrust for it or contempt for it?or jealousy, in my opinion, for it. There?s turf war issues even within plastic surgery.? Stevens adds. ?Now, outside the division, we?re trying to embrace the core.?

THE CORE

Stevens is quick to says it is ?ophthalmic plastics, facial plastics, derm-surg, and then aesthetic plastics, and we?ve come together formally and met and had core meetings,? he says. ?There is still a fair amount of teeth gnashing and discontent, but at least we?re talking. I?m in favor of the core. I know that certain political people are not.?

He believes that, as a group, plastic surgeons need ?to turn our eyes on the noncore?the dentists doing Botox, breast surgery, and liposuction; the general practitioners doing laser surgery. There are a lot of very undertrained people in this space doing things that they really have no business doing. Ultimately, the Hippocratic Oath is first we do no harm. It seems part of that is first we have training, knowledge, and experience. If the training is coming from equally unqualified people, that?s not training. I can hit a million golf balls and I?m not Tiger Woods, just because I hit a million golf balls. Well, I?ve hit all million poorly. When you listen to some of these nontrained, noncore people, they?ll say, ?Well, I?ve done it this many times, or I?ve learned it from this guy who?s done it this many times.??I don?t think the number of times you do it is the sole criteria. I think it?s the quality in which you do it. We owe it to the patient, to the consumer, to provide the best service of everything. To have people dabble in it, jump in and say [that] because they went to a weekend course at a hotel, [and say] I?m now a cosmetic surgeon. It?s wrong. You walk into your dentist; they?re selling you Botox. That?s crazy to me.?

This is a positive step toward keeping the plastic surgery within plastic surgery, then? Yes, he says, but, ?The ultimate goal really is to educate. We?re educating residents to be better plastic surgeons. I want them to see what we offer and also hopefully stimulate interest in that division? of plastics. To just be better, well-trained, well-rounded plastic surgeons. Even if they say they?re not going to do cosmetic surgery when they?re training, when they?re idealistic, the statistics are they will make a significant income from cosmetic procedures,? if the past is any indication, he adds.

The majority of them will generate more than half of their income from cosmetic surgery procedures, Stevens remarks, ?even though they?re wonderful hand surgeons, or microsurgeons, or craniofacial surgeons, or pediatric surgeons, or reconstructive surgeons. These are all divisions we get tested on. There is a commitment to test, in terms of the boards, to test for cosmetic plastic surgery knowledge base both written and oral, or practical, and so with that commitment by the American Board of Plastic Surgery will come better training around the country.?

He is encouraged and enthusiastic because he is seeing so many physicians who hold a commitment to train their residents. ?It?s an economic commitment, a time commitment, a physical space commitment,? he says. ?There will be some that go slower than others. Change happens with pain, and it?s a little slow at first and then it will pick up, it will accelerate, and then it will happen. We still haven?t gotten to 50% yet. It?s still at the beginning. It?s terribly exciting.?

SPECIALISTS UNITEA

Stevens looks into the future of his field, he sees integration. ?I see specialization and subspecialization within cosmetic plastic surgery. The two biggest changes I see are integration of the various subspecialties of cosmetic plastic surgery. Under one roof, facial plastics, derm-plastics, ocular plastics, aesthetic plastics, and traditional board-certified plastic surgeons.?

The people that embrace this rethinking of aesthetic medicine to accommodate the noncore practitioner under one roof?for example, cosmetic dentistry and OB/GYN?will excel, Stevens says.

Looking at the growth of all nonsurgical cosmetic procedures?nonsurgical body contouring and facial rejuvenation, for instance?Stevens sees huge growth. ?In those two areas, if you look at the growth over the last 20 years, 10 years, 5 years, 3 years? the acceleration has been phenomenal.

?If you track our own development back from 1988, when I had my first aesthetician, starting a medispa with existing space, and had a nurse do collagen injections. Eventually, [I] hired a PA and everything was growing. But there was no dedicated space. Then the PA became three PAs and then two aestheticians, and then three aestheticians. Facials and then more lasers and more energy-based technologies were added.?

Half of the technology Stevens has in his practice is less than 3 years old. ?I have 23 lasers and light-based technologies now. That?s incredible to me. If you just look at income stream, it used to be you provided this to optimize your patients. The river used to run from my surgery practice to my nonsurgery practice. The river has turned directions and is running from my nonsurgery practice to my surgery practice. It?s happening before my very eyes. I?m watching evolution. I never thought about the other ramifications. It started with optimization but then it became acquisitions, because some people wanted to have some facials and laser treatments.

?I never thought about the retention part, but now it?s grown into retention. If I provide the service, you?re not going to go to another physician to get it, because now you?re my trusted patient. You come once a month for your facials, and you come three or four times a year for your neurotoxin and your filler. When you?re sitting there for a half an hour waiting to get your filler, you?re being educated about what?s out there. A lot of people don?t even know what?s out there. So, then there comes the conversion.?

The American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) has awarded Dr. Keith LaFerriere the 2011 Larry D. Schoenrock Award for Distinguished Service. The award was presented last month during the AAFPRS Fall Meeting in San Francisco.

The AAFPRS grants the award annually to an individual who has made significant contributions to the facial plastic surgery program and its support within the specialty, organized medicine, and state legislative and regulatory boards.

Dr. LaFerriere has been involved with the AAFPRS and the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS) for years. ?Keith LaFerriere is part of the cadre of unsung AAFPRS heroes who devote an incredible amount of time every year to ensure that examination questions are reviewed and revised to meet rigorous psychometric standards,? says Ira D. Papel, MD, ABFPRS president. ?For the past seven years, this has been Keith?s way of cementing the Board?s reputation as the gold standard of certification for facial plastic surgeons?a cause that Larry Schoenrock proudly championed.?

LaFerriere was elected to the ABFPRS Board of Directors and has served as an examiner for 13 years. He also served as President of the AAFPRS in 2003-2004 and has been a fellowship director with Dr. Gregory Renner since 1995. In addition, he has served on the board of the International Federation of Facial Plastic Surgery Societies (IFFPSS) for the past eight years and has been involved in the development of its certification process and has served as mentor to international colleagues pursuing this new credential.

In addition to his work for the ABFPRS, the AAFPRS, and the IFFPSS, LaFerriere has also represented the specialty with his appointment to the Missouri Board of Healing Arts, the state?s licensing and disciplinary board for physicians.

Bullied Into Plastic Surgery? - KMGH Denver

According to the American Society for Aesthetic Plastic surgery, more than double the cosmetic procedures are being performed on children compared with statistics 13 years ago.In the last decade, the reports of teenage bullying in school have doubled.7NEWS has learned some teenage girls are opting for plastic surgery because of what bullies are saying in school.In old pictures 14-year-old Navana Britto points to one thing, her nose."There was like this huge bump right here," Navana said.That was before the eighth-grade student had Rhinoplasty a month ago."It was really obvious if you looked at me from the front," remembered Navana.But she said she really didn't have a problem with the way her nose looked until a bully at school said this, "Navana your nose looks messed up. Have you thought about getting it fixed?"Soon Navana was asking herself the same question."Wow I didn't realize it was that messed up. Do I really need that much work done? I wasn't comfortable in my own skin," said Navana."Nobody likes to see their child's feelings get hurt. She wanted to have it done, and if it will make her feel better we'll do it," said Vicki Britto.Vicki first agreed to surgery because she said her daughter had breathing problems from a deviated septum. Losing the bump her daughter was teased for, "Was a bonus as far as I'm concerned. She could've just gotten the inside fixed and just left the outside, but you know if it's available to you and will make you feel better, why not?" said Vicki.7NEWS asked Vicki if at 14 years old her daughter was mature enough to choose surgery. "Yes she was," answered Vicki."I'm very grateful that she had it done and I'm very grateful to Dr. Raval for doing it for her," said Vicki.7NEWS asked Navana's surgeon, Dr. Jeffrey Raval, 'How young is too young?'"It's a good question. It's hard to say," explained Raval. He said it depends on the child and the procedure."We do want to interview the older ones and make sure it's what they want. When kids are entering school and parents are concerned about teasing we can do something like pining the ears back as early as the age of 5 or 6. We've found at that age, that it is pretty close to adult size and it doesn't affect future development of the ears if we just pin them back a little bit," said Raval.Raval said he has seen more parents paying to make cosmetic changes for their children. One of his young patients was a 13-year-old girl who underwent Rhinoplasty and a chin implant."The surgery improved her self esteem and the way she sees the world. Cases like that I think we did the right thing," said Raval.7NEWS asked Raval how the decision to undergo surgery on the 13-year-old girl was made. "She made it with her parents. We met with the parents. We discussed everything. We went over all the risks and the benefits. We went over what to expect, what not to expect. We actually met with them several times and the parents with the child agreed that it was something they wanted to do," said Raval. "I think many kids would be grateful that their parents were able to do this for them."We showed the before and after pictures to an expert in child psychology."There is significant change clearly," pointed out Shawn Worthy. "That would be a decision I would want for my child to make as an adult, or someone else's child to make as an adult and not at such an early age," said Worthy.Worthy said cosmetic surgery on bullied children is, "A superficial solution to a broad and significant problem." His message to parents considering cosmetic changes for their children, "I would say rethink it. I would say really think about what you're saying to your child."Worthy is concerned about the message that he said a surgical fix sends."I'm not quite right. I'm imperfect. I'm not quite good enough. I don't look quite good enough. What message you know does that have to carry them through their lives for the rest of their life? That's the question that I wonder about," said Worthy.When teasing and bullying gets bad, Worthy said, "That's a time for a parent to step in and talk to the teachers, talk to the schools and do those things. But, I think plastic surgery is not the solution.""I do see where people are coming from where they say that this is controversial, but I do think that it is an answer for the right person, in the right family and it's a family decision," said Raval. "And everybody just has to decide for themselves if this is something that they want to do or if they want their child to continue to be teased. I'm not saying it's the best decision for everyone. Some kids will do just fine even though they're teased their whole life and they'll be much stronger for it, but some kids won't.""When he handed me the mirror I was like, I was just shocked," said Navana.Old pictures are all that's left of Navana's old nose. She is still a little swollen one month after the surgery. It could take up to a year for her nose to completely heal, but the bump she was teased for is gone."I'm really happy that this has kind of changed," Navana said as she pointed to the bridge of her nose.Navana said that since the surgery she is more confident, and the response at her school has been as smooth as her new profile. She said her old bully backed off and the teasing has stopped. But Navana said she is still carrying the hurt from the bully's mean words. "That's still in the back of my mind even though after I got it finished and everything," said Navana.A statement from the American Society of Plastic Surgeons says: "The ASPS has no formal position on plastic surgery for teenagers. As with any surgery, parental consent is required for all plastic surgery procedures performed on teens younger than 18. The ASPS advises parents to evaluate the teenager's physical and emotional maturity and believes that individual cases merit careful evaluation under the guidance of a plastic surgeon certified by The American Board of Plastic Surgery."
